Every year we set up a template to print our own 1099-MISC and 1096 transmittal forms, this year we realized we might as well create a template in Microsoft Word and share it with our fellow independent workers. This template will NOT print the actual 1099-MISC or 1096 form, but provides fields where you can fill in the appropriate information to be printed on the forms you can order from the IRS or purchase at an office supply store.
We’ve set these templates up with the fields that we typically use on our 1099-MISC forms for our creative contractors, however, depending on your business you may need to provide additional numbers in other fields we have not accounted for on this template. You can easily copy-and-paste the text fields and line them up for additional fields. Please make sure to reference your printed forms to be sure you have completed all of the fields required for your needs.
Depending on whether you use laser-print or dot-matrix forms, you may also need to adjust the placement of the fields because the margins of these forms vary slightly. The following templates were set up for use with 2013 dot-matrix 1099-MISC forms purchased at Staples. You will want to do a test print on plain paper to line up with printed forms before actually printing the information onto the final forms.
